I have also included a few helpful reminders.

At the bottom of the medical release form is an area where parents can list medical conditions (asthma, etc.), medications and other requirements (diet, allergies, etc.). This is very helpful to us because it is easier for us to help our students if something happens and we know about it, than if we don’t.

Please remember if a student goes to the doctor and will need to take any medicine here at school, we need to have a medication request form filled out by the parents and the doctor. These forms are available at the front desk, from me, in the student handbook, or on the forms part of the school web page.

Also please remember to fill out the asthma care if your child has asthma because that helps make it easier for us to help your child should he or she have any problems here at school.
